Anonymous feedback with a QR code
Anonymous feedback with a QR code gives people a way to speak up on their own phone. TellSafe generates a code for your organization's public feedback page, so you can put the same feedback link on signs, programs, and table tents.
Make your feedback QR code
- Create your TellSafe organization and check the name and public feedback address you want to share.
- Open QR Code & Link in the admin sidebar. Use the feedback destination for this sign; a survey or waiver is a different workflow.
- Download the QR image, or use the poster's print or download option. The QR code generator is included in the Free plan.
- Put your organization's QR image in the blank square on the downloadable sign below. Add the organization name, public feedback URL, reviewers, and expected review schedule before printing.
- Scan the finished print on more than one phone. Confirm the organization and feedback form are correct. Changing your organization's URL later breaks old printed codes, so reprint them if you change it.
Place it where people can pause
Use an eye-level sign by an exit, a table tent in a seating area, or a space on a printed program. Keep the code large, high contrast, and clear of other graphics. Leave white space around it and test at the distance people will scan from. Print the feedback URL too, for anyone who prefers typing it.
What someone sees after scanning
The code opens your organization's public page in the phone's browser. The person can open the feedback form, choose a category and privacy mode, write a message, and submit without creating an organizer account. Plain Anonymous mode needs no name or email. Where available, Anonymous Relay asks for an email for private follow-up; Identified mode shares the details entered. A QR code only opens a link: the selected mode and what someone writes determine what they share.
What to print so people can trust the sign
Name the organization and say who reads submissions and when. Suggested wording: 'Tell us what to improve. Choose Anonymous to send feedback without a name or email. Want a reply? If Anonymous Relay is available, your email stays hidden from organizers. Avoid names or details that identify you.' Use the relay line only if your plan supports it. Explain where to find updates and give a separate emergency contact route. Avoid absolute promises such as 'nobody can ever identify you.'

Frequently asked questions
Does the QR code make a response anonymous?
The code opens the public feedback page. The submitter chooses a privacy mode. Plain Anonymous needs no name or email; Identified mode shares the details provided, and relay uses an encrypted email address for replies.
Can I make a QR code on the Free plan?
Yes. The shared Free plan includes the QR code generator and plain anonymous submissions. Anonymous Relay for private follow-up requires Community or Pro.
Is the printable sign ready to scan?
No. It is a blank template. Insert your own organization's QR image, add its feedback URL and review details, and test the printed result before placing it.
